Hair/Scalp PRP Sign Up and Scheduling
Please review the following information to familiarize yourself with the process to book TitanX Men’s Health PRP appointments
You have two simple enrollment options to choose from:
We invite you to take advantage of our complimentary virtual Meet & Greet session. This brief 10-minute video consultation provides an opportunity to address any questions you may have and determine whether you’re a suitable candidate for PRP therapy.
If you’re already familiar with PRP therapy and are ready to proceed, simply click the “Sign Up” button to schedule your procedure at your convenience.
1) Complete The Intake Application
First, decide if you would like to Sign Up and Schedule Now or if you would like to Schedule a free Meet & Greet to get any questions answered.
After you have made your decision, you will be directed to the appropriate Intake Application.
This initial step involves gathering some essential personal and financial details from you. A payment method will be needed through our secure processor. You won’t incur any charges until the procedure/services are rendered in the office.
2) Schedule
Whether you’re scheduling a complimentary Meet & Greet or a PRP treatment appointment, our automated enrollment process will guide you to the appropriate scheduling calendar.
If you do not see available time slots or require alternative scheduling options, please contact TitanX Men’s Health directly.
3) Complete Your Forms
Once you’ve signed up, TitanX Men’s Health will send you three electronic forms to complete:
1) Patient Agreement
2) Medical History Form
3) Procedural Consent Form
It is essential that all forms are completed electronically prior to your neurotoxin procedure.
4) Be Sure To Review Our Patient Care Instructions
Familiarize yourself with TitanX Men’s Health preprocedural and postprocedural instructions.
