Once you’ve scheduled an appointment, TitanX Men’s Health will send you some electronic forms to complete This may include:
1) Patient Agreement
2) Medical History Form
3) Procedural Consent Form
It is essential that all forms are completed electronically prior to your neurotoxin procedure.
