If this is your first cosmetic toxin treatment with TitanX Men’s Health, please schedule a TitanX Toxin (New Patient) appointment.
If you are an existing patient and have previously received toxin treatments at TitanX Men’s Health, please schedule a TitanX Toxin (Established) appointment.
This initial step involves scheduling and gathering some essential personal details from you. A payment method will be required through our secure processor. You won’t incur any charges until the procedure/services are rendered in the office.
Once you’ve scheduled an appointment, TitanX Men’s Health will send you some electronic forms to complete. This may include:
1) Patient Agreement
2) Medical History Questionnaire
3) Procedural Consent Form
It is essential that all forms are completed electronically prior to your neurotoxin procedure.
Familiarize yourself with TitanX Men’s Health preprocedural and postprocedural instructions.
This appointment should be scheduled only after receiving a cosmetic toxin treatment with TitanX Men’s Health, preferably 10–14 days post-procedure.
